Abstract  Treatment of cording (ZVK-Wissenschaftspreis 2007)

Abstract: This study examines the effectiveness and possible complications of a new treatment approach for patients with axillary cord formation following breast cancer surgery.

The cord can be visible or palpable at the axilla and can extend beyond the elbow as far as the hand. In some cases the cord can be seen in the chest. The cord has been found to arise due to the fibrosis of lymph vessels severed as a result of lymph node removal, which can become attached to the scar tissue. The phenomenon is also referred to as “fiddle-string” phenomenon and axillary cording. Cord formation results in painful flexion and abduction mobility restrictions of the ipsilateral shoulder, which are usually treated by means of active exercises.

Manual (massage) techniques have previously been regarded as contraindicated due to the risk of lymphedema.

In the period from December 2003 to September 2006, a study was made of 123 patients with cord formation and/or surgery-related mobility restrictions of the shoulder following breast cancer surgery. Cord formation was found in 105 of these patients.

The restriction of mobility was documented using standardized measurement methods, and visible cords were photographed. By means of special manual releasing techniques, the cord was completely remedied in 94% of the patients, with an average of 9 to 12 treatment sessions; considerable improvement was achieved in the remaining 6% of the patients. Arm mobility restrictions were completely eliminated in 90% of the patients, and were significantly improved in the remaining 10%. The method is effective even in the case of cords and mobility restrictions that have existed for many years.

It is demonstrated that the described treatment did not lead to any side effects such as lymphedema.
